All visits to the Student Health Centre are completely confidential. No
information from a student's visit or health record can be released without
explicit and informed consent of the individual student. The student must sign a
consent form and provide picture ID (student ID card or driver's license) prior
to release of any information. This is to ensure the appropriate individual is
in fact consenting to release the information.
